Certified Care Manager Services Include:
- Geriatric Care Management
(Healthcare Advocate)
- Hospital visit before dismissal
Physician/physician office coordination
- Senior Care Planning
- Financing Care – Options and assistance through Medicare, Medicaid, VA, community services
- Elder Cognitive Functioning Assessment
- Home Safety/Security check and recommendations
Nursing Services
- RN Staff Visit includes health assessment and monitoring vital signs, blood sugar check, in-home lab work, Coumadin (blood-thinners) monitoring, medication monitoring/fill med box or automated dispenser
- Medication Monitoring Visit— Fill medication box or fill automated dispenser (every 2 weeks or monthly)
- Lab Work Visit—venipuncture draws or finger-stick, Coumadin (blood-thinner) monitoring
- Daily Home Vital Sign Monitoring Station
(Weight, blood pressure, pulse, blood oxygen saturation monitored by an RN daily using state-of-the-art technology via telephone line) - In-Home Bathing/Dressing Assistance
- Physical Therapy — home rehabilitation/ therapy for continued recovery
- Counseling for — bereavement and grief, depression, anxiety, caregiver stress
